After concluding the last book, quite literally, with a bang, Mike's first year of residency continues. Much of what had seemed settled is suddenly placed in disarray. Doctor Gibbs, widely thought to be a shoo-in as the next Chief of Emergency Medicine, is rendered incapable of performing the necessary duties due to her injuries. This results in an outsider being hired for the role, one who needs to adjust to this new rural hospital just as much as his new charges need to adapt to his more urban and militaristic style.
That's far from Mike's only battle, though. Much of his remaining year is spent dealing with legal issues, trying to defend himself and colleagues from a surgical encounter that resulted in a poor outcome for the patient. Then there's the issue of the nurse's contract, which causes its own level of strife, straining the relationship between nurses and doctors. Additionally, Mike is presented with an offer that seems too good to refuse, but also conflicts with both his existing Residency program and his personal ethics.
However, it's not all doom and gloom. Kris is still pregnant as we open our story, and there will be a bundle of joy delivered part-way into the story. The long-awaited new Emergency Department also debuts, vastly expanding the hospital's potential. Even with all the aforementioned obstacles, right now, Mike's life is (mostly) following his expected course. The only question is, what comes next?
